Aspirus Health in Wausau, WI is seeking a Registered Nurse – Care Coordination Lung Navigator to join our team.

The RN Care Coordination Lung Navigator ensures timely access and seamless coordination of services throughout the continuum of care for a designated patient population which includes patients presenting for lung cancer screening as well as management of incidental lung nodules. The RN Navigator assists in coordinating timely diagnostic testing, referrals, and follow-up care if necessary. The RN Navigator is responsible for facilitating communication between patients, providers, and specialists to ensure continuity of care. The RN Navigator educates patients and families about disease processes, treatment options, and available support services and intervenes as needed to address barriers to care. The RN Care Coordination Lung Navigator functions as an active health advisor, educator and advocate throughout the process of screening, diagnosis, and treatment of cancer or other conditions. This position shares knowledge with patients and families regarding internal and external support services and resources, and tracks data on patients to ensure completion of recommended treatment plans and follow-up care. The RN Care Coordination Lung Navigator works directly with the health care team to deliver innovative health management practices to the patients and community of Aspirus.

HOURS: Full-time .8 or 64 hours every pay period. Day shift.

Experience/Qualifications

  • Bachelor of Science Degree in Nursing preferred
  • Possesses an active Wisconsin or Michigan RN nursing license.
  • Skills in organizing and prioritizing work along with multi-tasking simultaneously.
  • Skills in working effectively as a dedicated, creative and resourceful team player.
  • Possesses excellent oral and written communication skills as well as excellent interpersonal skills.
  • Demonstrates sound clinical judgment, decision-making, and problem-solving skills.
  • Willingness to be a change agent and positive role model.
  • Knowledge of motivational communication techniques to engage patients/families.
  • Ability to demonstrate a strong, autonomous work ethic with qualities of personal and professional integrity.
  • Detail and goal oriented demonstrated by personal initiative and accountability.
  • Possesses confidence to communicate and outreach to other community health care organizations and personnel.
