ASPIRUS ST. LUKE'S HOSPITAL in Duluth, MN is seeking a RN- ACUTE CARE COORDINATOR to join our CARE COORDINATION team!

Salary Range: $37.64 - $54.56 Hourly
Schedule/Hours: DAY/PM (EVENING) ROTATION, 8 hours
Weekend Requirement: Occasional Weekend
FTE: OCCASIONAL, 0.001 FTE


The RN- Acute Care Coordinator is responsible for assessing patient needs and coordinating referrals to resources to meet those needs. The RN- Acute Care Coordinator uses nursing practice knowledge in the development of the patient's plan of care. As part of the multidisciplinary team, the RN- Acute Care Coordinator works closely with physicians, nursing and ancillary departments, and provides support and clinical expertise to nursing personnel regarding patient care coordination issues. The RN- Acute Care Coordinator attempts to resolve social, economic and environmental difficulties that interfere with the patients' ability to manage their health needs effectively. The RN- Acute Care Coordinator requires independent judgment, analytical, problem solving, and collaborative skills in order to organize and prioritize multiple tasks to provide patient centered care.

Experience/Qualifications

  • Knowledge and skills normally acquired through completion of a Associates Degree in Nursing. BSN preferred.
  • Minimum of one year acute care hospital experience. Experience in critical care, general medical and/or surgical nursing, and experience in educational presentations beneficial.
  • Previous acute care hospital experience preferred. Acute care hospital internship recommended
  • Possesses current RN licensure, in the state of business units assigned.
  • Exhibits strong human relation skills for interacting with patients, patient's family members, physicians, personnel, community agencies, and other resources.
  • Displays analytical skills in order to assess patients' needs, develop associated discharge planning, and provide sound recommendations and guidance. Displays ability to concentrate and pay close attention to detail when interviewing patients & preparing discharge plans.
  • Possesses planning and organizational skills and expert skills in prioritization.
  • Understands the nature of social diversity and oppression with respect to race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, age, marital status, political belief, religion, immigration status, and mental or physical disability.
