Aspirus At Home in Crystall Falls, MI is seeking a PATIENT CARE COORDINATOR to join our field team!

The PCC provides clinical and administrative supervision to the Home Health location in addition to functioning as a Home Health Registered Nurse.The Home Health PCC is responsible and accountable for the delivery of safe and high-quality home health services and is an advocate for the patients under their care.

HOURS:Full Time; 1.0 FTE or 80 hours per pay period. Day Shift.

Experience/Qualifications

• Knowledge of patient care standards and practices normally acquired through completion of a Bachelor's Degree or Associate Degree from an accredited school of nursing.
• Possesses an active Michigan RN nursing license.
• Current BLS certification or obtained within 90 days of hire.
• Valid driver's license required.
• Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.
• 1 or more years' experience in home health care preferred
• Supervisory or management experience desired.
• Able to handle stress and adverse conditions calmly, pleasantly, and politely.
